What is premature ovarian failure?

Also known as primary ovarian insufficiency, it involves the loss or alteration of the common function of ovaries before 40 years of age. This means that “normal” quantities of the estrogen hormones are not produced, and the eggs are not released regularly as they normally would. This leads to irregular or missed periods, resulting in infertility in most of the cases.

Chromosomal defects, a cause of premature ovarian failure

Even though in most of the cases the cause is unkown, sometimes is related to certain genetic disorders such as Turner syndrome or the Fragile X Premutation. Fragile X is caused by mutation in the FMR1 gene, which is on the X chromosome. There is segment of this gene that it is repeated a variable number of times. In most people, this segment repeats five to 40 times.

When the segment in FMR1 gene is repeated 55-200 times, it is called PREMUTATION. When it is repeated more tha 200 times, causes Fragile X syndrome. Its symptoms include intellectual disability, behavioral and learning challenges, and various physical characteristics.

Premutation in FMR1 gene is the most common familial genetic cause of Premature Ovarian Failure. Premutated women are not only at risk of having Premature Ovarian Failure but also to have a child with Fragile-X syndrome since their mutation in very unstable and can expand (increase number of repeats) in next generation.
